What are Membership Plans?
As a member of the Orem Family Dental Care, you and your family will receive regular preventative cleanings, examinations and discounted dental treatment.
PLAN GUIDELINES

The membership fee is due at enrollment, valid for one year, and is non-refundable.

Orem Family Dental plans cannot be combined with dental insurance.

Payment for discounted treatment is due on the day of service.
